Low back pain is the second most frequently reported diagnosis, after the common cold, which is seen in outpatient primary care clinics. In the majority of cases, the symptoms are self-limiting and resolve by 6-8 weeks time. However, symptoms of back pain do frequently recur and, at times, can become chronic and develop into radiculopathy or sciatica. There are a variety of factors that can predispose someone to developing recurrent back pain such as a strong family history, trauma, certain occupations, smoking, obesity, inactivity, stress, and structural abnormalities.
Initial therapeutic intervention should stress patient education on the natural course of acute and recurrent low back pain, guidance regarding activity level and modification, the use of analgesics and other modalities. In moderate to severe cases, physical therapy as well as spinal injections may be indicated.
The goal of physical therapy is multifaceted and includes: - making patient aware of proper ergonomics and body mechanics - postural retraining - improving flexibility - core strengthening - addressing the fear of movement - symptom management with application of heat, cold and education on pacing, and back and sleep supports
In cases of moderate to severe pain which is unresponsive to conservative treatment, spinal steroid injections, performed with the help of x-ray guidance, can alleviate the pain of a herniated disc, spinal stenosis and lumbar facet joint synovitis.
